Please take note of the following format and submittal requirements:

• Please use this form for your nominations. Additional pages can be included as necessary. Also feel free to include photos.
• The name of the award category must be included with the nomination submittal.
• The person submitting the nomination will receive a confirmation email indicating the nomination has been received and reviewed for completeness.
